Support Coordinator (QP)

Description: Full-time Support Coordinator (QP) responsible to develop and coordinate Plans of Care for individuals with Intellectual/Developmental Disabilities.  Duties include meeting with customers and families to address needs and concerns; complete all documentation in a timely manner; provide supervision to Direct Care Staff including review of staff documentation and work strategies, provide one-on-one supervision as needed; implementation and review of customers Plan of Care.  Must also be willing to work cooperatively with community partners as well as staff from other InReach departments, respond to all needs and requests in a positive and professional manner, be available to provide on-call service for emergencies.
Qualifications: Must have Qualified Professional status; (4-year college degree in human service field plus two-year’s supervised habilitative experience working with special needs population); possess a valid driver’s license and maintain safe driving record.
Location: Charlotte, NC 
Schedule: Mon – Fri between 8:30am – 4:30pm, hours may vary depending on customer needs.
