How to Access Our Services
InReach offers a broad range of supports and services for individuals with intellectual and developmental disabilities, as well as their families, throughout their lives. Services are provided in various settings depending on the individual’s needs, funding requirements, and, in some cases, housing availability.
If you are on the waitlist for the Innovations Waiver or the TBI Waiver, you can get services while you are waiting. You will NOT lose your spot.

Acessing our services overview
Step #1
CONTACT CArE MANAGER
Care Manager completes an independent assessment and submits it to the state.
Step #2
CARE PLAN | Indvidual support plan (ISP)
The Care Manager, care team member, and family develop a Care Plan/ISP.
Step #3
STATE CONDUCTS EVALUATION
THE STATE OF NC WILL CONDUCT AN INDEPENDENT EVALUATION.
( You will receive a letter regarding your eligibility.)
Step #4
FOLLOW-UP
THE CARE MANAGER SUBMITS THE COMPLETED CAREPLAN/ISP FOR AUTHORIZATION AND FOLLOWS UP WITH THE PROVIDER AGENCY (LIKE INREACH) ON SERVICE DELIVERY.
